AD&D Important Information

Important information about accidental death and dismemberment coverage:

  1. Once you are eligible, you can enroll in accidental death and dismemberment (AD&D) insurance anytime during the year, including during annual enrollment.
  2. Evidence of insurability is not required for coverage.
  3. If accidental injury results in body dismemberment or loss of use or loss of sight, speech, or hearing, the plan will pay according to the extent of the loss. If accidental injuries result in death, the full benefit amount will be paid to the beneficiary.
  4. Deductions for AD&D insurance coverage are made on an after-tax basis every pay period. If the deduction isn’t made because you don’t work enough hours, the cost of the coverage will be deducted from your next available paycheck unless you live in CA, CT, DE, MN, NV, or VA. If you live in one of these states or you are on an approved leave of absence, you will be billed at your home address on file. In addition, if you live in one of these states and your coverage was terminated for non-payment, you will be required to pay unpaid premiums before you re-enroll in the plan.
